The objective of case management is designed to help you accomplished the decision that you made during your next step assessment and evaluation.

We often times have a vision, plan, or objective but we have no way to carry out or accomplish what ever it is that we have a desire to do. The quote “If there is a will, then there is a way”. Must hold true to the way of thinking and the approach of life during this period of life that we are now in.

Case Management is design so that you can sit down with your case manager and review all the options that were presented and then help you facilitate, structure or firmly establish what your next step is going to be.

There may be those that have no idea how to fill out college enrollment forms, trade school applications, certifications request, social security, unemployment or disability.

Case Management is here to not only assist you, but to help you learn how to assist yourself in the future. This is why this program is called Case Management and you, because it takes two to accomplish the end results of this phase of the project.

Case Management is not here to do the work for you, but to help you get the work done.  

Case Management does not end once you are enrolled in school or employed in another trade or job. Your Case Manager and our organization is interested and equally concerned about your success as well as our success ratio in helping people.

We want to make sure that we establish a means of follow up communication.
